SUPPORTING STATEMENT: I am a volunteer with Grimsby Cleethorpes and District Samaritans, this is a registered charity. I have volunteered for the charity for seven years and have a good appreciation of emotional health issues which are both relevant and significant within North East Lincolnshire. I am currently publicity officer for Grimsby Samaritans but I have also held the post of deputy director. In my post as publicity officer I attend a number of local meetings with partner organisations and aim to raise awareness of emotional distress and suicide. I was nominated for local volunteer of the year awards in 2009. I also sit on the management committee of North East Lincolnshire Lesbian, Gay, Bisexual and Transgender Forum (LGBT), working with other members and partners to support members of North East Lincolnshire LGBT community.

SUPPORTING STATEMENT: As an individual I joined my group to help people, this is something that may help all ages and children and to help all ethnic origins and need more physical financial support. I have two grandchildren with autism so I have much interest in this and working in the Nunsthorpe Bradley Park Residents and Tenants Association. I work in our shop as a Volunteer. I meet many people of all ages and they tell me all their ills and pains so I want to improve their lives and try to help the people of North East Lincolnshire to get their entitled from the Government so it improves the lives of people of our estates and Schools and across the board of all this that helps the lives of everyone.

SUPPORTING STATEMENT: I feel as part of the North East Lincolnshire Drug & Alcohol Action Team (DAAT) managing the Roundabout User Group, which is a group I set up along with my deceased ex partner to support advice and guide anyone suffering from substance misuse of Alcohol problems, whether past or present as well as changing and shaping for the better the drug Treatment, Criminal Justice, Health and Social Care such as access to Dentists, Hepatitis C support, Dual Diagnosis, Carers and Young Peoples Centres/Support Police, Probation, Local Communities and Local Partnerships, Housing, Training and other 3rd sector and Voluntary Agencies that work are connected with this client group and linking in with the National Treatment Agency (NTA) on issues, trends and any concerns/problems/compliments. Due to the success the support group gained Regional recognition for its model and its working. I also have personal experience for someone with mental health problems substance and health problems and I feel my experience, knowledge and keenness to help change and form future services would be beneficial for the group and myself both professionally and personally.

SUPPORTING STATEMENT: In January 2007 the Department of Health (DOH) announced the schemes that had been successful in applying for inclusion in Department’s pathfinder programme for Social Enterprise. A new Grimsby project designed to meet the Health and Social Care needs of some of the most vulnerable groups in society have received ministerial approval. The Door Open project beat off competition from 5000 other applicants to be one of 22 projects nationally to be awarded “pathfinder” status by Ivan Lewis, Minister of Health. Open Door was set up as a local social enterprise and will provide a board spectrum of services aimed at improving quality of life. Social enterprises are dynamic businesses and social and environmental aims. Other well known examples include the Big Issue, Cafe Direct and Jamie Oliver’s Fifteen. The local project provides a wide range of key services including general medical services as well as nurse-led Health and Social Care Support Team. The aim is to bring specialist provision into the most deprived local communities in order to improve access to essential services for some of the people in need. We have received national recognition for its innovative use of a Design Engineer who has previously worked for the design council to work alongside potential services users to design holistic Health and Social Care Service.

SUPPORTING STATEMENT: I have campaigned for disability issues in North East Lincolnshire and I am a member of the Focus Group for Disability Engagement Strategy (DES) document of North East Lincolnshire Council; having taken the Council to task for not having a document that conforms to the law. I am a member of North East Lincolnshire Disability Access Group (NELDA) and undertake briefs for them to confront discrimination and illegal practices of business and organisations against disabled people. I am a co-coordinator of the disabled parking reporting initiative to Grimsby Police.
